Having read the Activity Center Rules & Regulations (pages 1 & 2), I (We) acknowledge that a Violation of these rules can result in termination of privileges at the sole discretion of the SPOA Board.KEY CARD REQUEST FORM
ACTIVITY CENTER HOURS: 5:00 AM -10:00 PM
The ALARM is set at 10:00 PM - No Exceptions
Steps that can be taken:
(1) Verbal or written warning.
(2) If the same violation occurs again, the Key Card will be turned off (inactivated)
(3) A written appeal will need to be submitted to the SPOA Board before the Key Card is re-activated.
A SIGNED FORM IS NEEDED FOR EACH SPOA PROPERTY
